Job Description:
A company is looking for an Annual Wellness Visit Specialist -
LPN or MA in Population Health.
Key Responsibilities:
Conduct non-provider components of Medicare's Annual Wellness
Visits (AWVs) using standardized tools
Coordinate follow-up health care services and assist in Advance
Care Planning
Document all patient interactions in health records and participate
in quality improvement activities
Required Qualifications, Training, and Education:
AA degree
Iowa state licensure as a Licensed Practical Nurse (LPN) or
Certified Medical Assistant (CMA)
Basic Life Support (BLS) certification required
Minimum of 3 years of health care experience
Completion of Mandatory Reporter training within three months of
hire
