Job SummaryParrish Healthcare is seeking a skilled Case Manager Social Work to join our team. In this role, you will work closely with patients and families to provide comprehensive social work services, including psychosocial assessments, interventions, and discharge planning.Key Responsibilities

• Assess patients' and families' psychosocial risk factors and develop interventions to address identified needs.
• Collaborate with healthcare teams to provide support and resources to patients and families.
• Develop and implement discharge plans that ensure a timely and safe transition for patients.
• Provide crisis intervention services as needed, including support for patients and families experiencing emotional, social, and financial challenges.Requirements
  • Master of Social Work (MSW) degree from a Council on Social Work Education-accredited program.
  • Licensed Clinical Social Worker (LCSW) preferred.
  • One to two years of hospital social work experience or comparable clinical experience.Benefits
